At Exemplar Health Care, our mission is to make every day better for adults living with complex needs. Our services in Castleford enable people who would often be in hospital or a higher acuity setting, to access complex nursing care in a community based home. Our holistic approach to care means people get the right care from the right team, first time round. With this approach, we aim to stop the cycle of failed placements and reduce readmission to acute settings.
Fairburn Vale complex needs care home in Castleford, West Yorkshire, specialises in supporting adults living with neurological conditions including brain injuries and strokes, complex mental health needs and physical disabilities. Our Nurse-led team has the skills and expertise to support people with complex needs including prolonged disorders of consciousness and behaviours of concern, and those who require tracheostomies, ventilators and PEG feeding tubes. We support people on their rehabilitation journey from hospital or living in a secure setting to community-based living. We also provide long term support for people living with degenerative or life-limiting illnesses.
