Overview

Granby Intermediate Care Hub is owned and operated by Liverpool City Council, and is part of their Department of Adult Social Care & Health.

The Hub is a single level (ground floor) property in the Granby / Toxteth area of Liverpool. It has 30 en suite single rooms dedicated to short term reablement care* of its residents, the building has its own small garden areas accessible internally to each of its three units. Each unit has its own dining area and lounge for the ten residents within that unit. There is also a light and airy central atrium where large group activities, physiotherapy assessments, and visitors can sit.

The facility has on site physiotherapists, occupational therapists, and social workers during weekdays, and reablement staff 24 hours a day, 7 days a week. Nursing care is only provided on referral for specific reasons from the discharging hospital.

The kitchen staff provide 3 meals per day, and alternative meals can be provided for residents with alternative dietary requirements.

If you are visiting a relative at Granby Hub, our visiting times are 2pm - 4pm, and 6pm - 8pm. Visiting outside of these times should only be made in exceptional circumstances, and with prior agreement with the Hub.

*Reablement Care - the purpose of Granby Intermediate Care Hub is to transition service users from Hospital (or pre-hospital admission) back home or to their next permanent residence. This is completed via directed therapies, social work interventions and assessment for post reablement care and equipment.
