Overview
Two Rivers is a home in the area of London, which can accommodate just over 5 residents. Care can be provided for those with dementia, learning disabilities and mental health needs.
•To provide an outstanding quality, caring and therapeutic environment for vulnerable Asian women with intellectual and/or physical disabilities, and/or mental health issues, using highly experienced, dedicated, and caring staff to help and encourage residents to become more independent.
•We aim to support our service users to increase their independence using elements of the recovery model, with the objective to, wherever possible, facilitate moving onto step-down or independent living within the community. We will work closely with families, social/health care professionals and carers towards greater independence. We aim to move people on as soon as they are ready and will take all steps to build independence quickly, including proactive and targeted training, group work and therapeutic interventions.
•A person-centred approach to care will be used. We will ensure that all service users have their own Health Action Plan and are registered with a GP, dentist, chiropodist and any other appropriate health professionals. We will support and help service users to develop coping strategies and build self-awareness. We will train staff to identify relapse indicators and to keep a close eye on the well-being and health of all residents to avoid further admission into the hospital.
•To minimise the risk to vulnerable adults by ensuring that all staff have undertaken in-house training in safeguarding, mental capacity, managing challenging behaviour and all appropriate health and safety training (first aid, hygiene, fire). Central to the home’s aim and objectives is the promotion of the individuals’ right to live a fulfilling, meaningful life which is inclusive both in and home and the community.
•We aim to provide continuity of support to service users once they have moved on to supported or independent living. Two Rivers is registered to provide personal care without accommodation as well.
