Overview
Preston Private Care Home was purpose built in 1987 housing up to 100 individuals. We support and care for up to 100 older people in four separate units; Ladywell, Longsands, Durton, & Fernyhalgh. We care for the people who require Social Care, Nursing Care and Specialist Dementia Care.
Our home has been thoughtfully designed to suit the varied needs of the people who live here. It is a modern purpose-built bungalow type building with all facilities on the ground floor, giving wheelchair access. The building is spacious, light and airy. Those who live here are supported to decorate their rooms according to individual preference and taste.
The landscaped gardens offer beautiful views of the Lancashire countryside and provide a peaceful atmosphere for those who live here, their family and friends to relax in.
OUR PERSON-CENTRED APPROACH
At Preston Private Care Home, we are committed to delivering care that is tailored to individual preference. Our person-centred practice ensures that you, or your loved ones receives appropriate support.
PERSON-CENTRED PRACTICES AND TECHNOLOGY
To support our approach, we use specialised tools and practices, including a digital system called Person Centred Software. This advanced system helps us document care notes and manage care plans effectively, ensuring all information is accurate and up to date.
Note: Our colleagues use handheld devices that may resemble mobile phones but are, in fact dedicated tools for documenting care.
KEYWORKERS AND NAMED NURSES/SENIORS
Those who live here are assigned a Keyworker and a Named Nurse or Senior, who will be your primary point of contact. They play a vital role in keeping you and your loved ones informed and ensuring that your care needs are consistently met. On admission day, you will be informed of the names of your Keyworker and Named Nurse or Named Senior. They will be happy to introduce themselves and answer any questions you may have.
PERSON-CENTRED CARE PLANS
We create individualised care plans for every resident.
These plans detail your unique needs and preferences, as well as the support required to meet those needs. Care plans and risk assessments are regularly reviewed and updated to ensure they always reflect your current requirements.
QUARTERLY CARE REVIEWS
We value collaboration with those who live here, their friends and families and invite you to participate in quarterly care reviews. During these reviews, we assess care needs, address any concerns, celebrate successes and discuss ways to improve.
BEDROOMS
Your bedroom at Preston Private is your personal space, designed to reflect your tastes. From wallpaper and artwork to furniture and décor, we encourage you to personalise your room to make it feel like home.
VISITORS AND FAMILY
We welcome family and friends at any time and encourage them to feel at home during their visits. Light refreshments and drinks are always available, and we are delighted to have visitors join us for special occasions. At Preston Private, we believe that care is not just about meeting needs. it's about creating a comfortable, supportive, and meaningful environment where everyone feels valued.
HEALTHCARE NEEDS
At Preston Private Care Home, we are dedicated to providing holistic care that nurtures the body, mind and spirit.
Our mission is to protect, inform, support, enable and empower everyone who lives or stays with us. Compassion and respect are at the heart of everything we do and we strive to ensure that every person feels valued and cared for at all times.
Those who live on our Nursing Unit benefit from 24-hour onsite nursing care, ensuring immediate support whenever needed.
Those who live on our Residential Units, nursing care will be provided by the District Nurse Team with whom we have a close working relationship. This collaborative approach ensures that every resident receives the appropriate level of care tailored to their individual needs.
Those who lives here are encouraged to register with our GP surgery as they can facilitate home visits when needed. If you wish to keep your own GP - we are happy to liaise with them
SERVICES
We provide a number of services such as Chiropody, Optician and Onsite Hairdressing for our service users. Please note that charges apply to these services and a list of services and prices is available in the Unit Offices. For further details or to schedule a visit or an appointment, please reach out to the Unit Leads.
We are proud to have a team of talented Activity Coordinators onsite who are dedicated to creating enjoyable and meaningful experiences for our those who live here. We offer a wide range of activities to cater to various interests and preferences. The Activity Calendar is available on the activity notice boards, where you can find details about upcoming events and programs.
