What is a Reconnect community?
Our Reconnect communities offer genuine inclusion by supporting residents to remain independent, active and engaged in daily activities of their choosing. We've adapted our Sunderland care home to include facilities you may find in many people’s homes; such as DIY work rooms, laundry facilities, arts and craft rooms and gardening equipment - perfect for our green-fingered residents!

Our aim is to support people living with dementia to reconnect with their past and feel happy in their new home, whilst our fully-trained carers and nurses assist with daily care requirements. To ensure your loved one receives an exceptional level of care, our Reconnect communities have a higher ratio of carers, nurses and dementia support workers; all of which have experience in caring for those living with dementia.

Our Reconnect communities encourage positive involvement, giving people a voice and supporting them to live a fulfilled life. We put together comprehensive, individualised care plans to support their medical, physical and emotional needs, whilst working with ground-breaking technology, such as PainChek, to help monitor the residents wellbeing and any behaviour changes
