Home to Decide- Taking the Crisis out of Care

Home to Decide reduces unnecessary care home admissions, saving money for commissioners and providing a better quality of life to those on the service. The service lasts up to 14 days starting at an intensive level and reducing in line with the individual’s progress.

To date, an amazing 67% of people completing the service have been able to continue to live at home. Our confidence in delivering outcomes enables us to share the financial risk.

The cost of the individualised one-to-one service will be capped at the residential care weekly rate. If the person remains at home, savings are shared for 10 weeks, if they enter residential care, there is no further cost.

For most people, living as independently as possible at home is preferable to going into residential care. But sometimes factors including mobility, health conditions and cognitive impairment can make staying at home challenging. Hilton Nursing Partners want to help meet that challenge enabling people to continue to live in their own home.

This unique service involves working in partnership with each individual to optimise their physical and emotional wellbeing, review their physical surroundings and to identify what support is needed and when. Home to Decide supports people to live independently and safely in their own home, saving money and reducing system pressures.

Commissioners only pay for successful outcomes. The charge for anyone who requires residential care is capped at the weekly residential rate. That makes it cost neutral if the person would have been in a care home and represents a significant saving if they would have been in hospital.
