Living Horizon is a CQC-registered specialist provider of 24/7 supported living for adults with forensic and complex mental health needs. We have done one thing since 2012 and we have stayed within it. Rated Good by the Care Quality Commission — and Good in all five key questions: safe, effective, caring, responsive and well-led — we support men and women aged 18 and over across eight homes in Aylesbury and Oxford, with a team of around 60, and we are expanding. We are a national provider: we accept referrals from anywhere in the country and will travel for the right placement. Every home is staffed around the clock, including waking nights, with a named House Leader based in it and hands-on senior leadership behind them.

We are often the last specialist option before a return to a secure setting or homelessness: people stepping down from low, medium and high secure hospitals; leaving custody or subject to Section 37/41 restrictions, MAPPA or probation oversight; on Section 117 aftercare with a history of repeated readmission; and people whose previous placement has broken down. Dual diagnosis is core to our work, and we hold mental illness alongside autism, a learning disability or an acquired brain injury rather than handing a person back because of a second diagnosis. Where care arrangements amount to a deprivation of liberty, we take the placement and run the Court of Protection process.

Our approach is boundaried, compassionate, person-centred and strengths-based — positive behaviour support, formulation-led risk management, and close working with the multidisciplinary network around each person: responsible clinicians, social supervisors, community mental health and forensic outreach teams, probation and MAPPA. We notice when medication is not right and we push for review. Everyone we support holds their own tenancy and lives in their own home, with support built around them.

Our stepped pathway — crisis, supported living, independent living — is designed to reduce support over time and build genuine independence, even where that ends the placement. We work with eight local authorities and two NHS bodies and assess in person within 48 hours, Monday to Friday, with a full risk assessment and management plan to follow.
