About Us
Destiny Integrated Care provides bespoke, person centered care packages carried out by expertly trained team of Carers and support workers who support and respect service users’ decisions and promote choice in the Domiciliary Care, Supported and Complex Care areas. Our aim is to help people with mental health challenges and learning disabilities to live as independently as possible. We currently have several placements around Bedfordshire, Peterborough and Cambridgeshire.
Our staff is dedicated and highly trained. They uphold the company culture of continuous improvement. We are a learning organisation, ensuring the right levels of training be able to give the right levels of support.

Domiciliary Care
We offer support for service users in the comfort of their own home by our expertly trained carers. Our services allow clients to live comfortably within their homes while still receiving assistance to achieve their assessed needs. We offer high quality support tailored to suit your needs at your convenience in the following areas:

Supported Living
We work hand in hand with the Community Recovery Teams to support service users with mild to moderate mental health issues to live independently. We offer shared accommodation, where everyone will be having their own bedrooms, shared lounge, kitchen and bathroom.
Our staff is fully trained in these areas, and we offer 24-hour around:
• Personal Care
• Continence Care
• Medication Management (Registering with GPs, Booking and accompanying to medical appointments, ordering, and administering medication)
• Supporting their Sensory needs as per recommendations by professionals.
• Managing their Dietary requirements (Meal planning, shopping, and preparation)
• Maintaining social interactions through various activities of interest.
• Managing finances (DOSH, Budgeting)
We have Support workers on site 24/7 to offer support with but not limited to:
• Medication management (Prompting and ensuring medication is taken on time and the right doses, monitoring, and ordering medication, booking medical appointments, and accompanying to the appointments)
• Maintaining emotional and psychological wellbeing (Engaging and interacting with them, reassuring them)
• Maintaining adequate Dietary and Fluid intake as in their care plans (meal planning, shopping, and food preparation)
• Maintain Personal Hygiene and Appearances (Prompting and Encouraging)
• Managing finances (Budgeting)
• Maintaining Social Interaction (Support this individual to participate and be involved in activities they are interested in and maintain relationships with family and friends)
• Assist with applications for Benefits.
Hospital Step downs
We provide hospital step down beds where we assist service users with mental health challenges to be reintegrated back into the community after being discharged from the hospital. These beds are offered in a shared accommodation where each service user has their own bedroom. A trained support worker is available at the house 24 hours to offer support with the following activities;
• Supervision of mental health, Monitoring and support with medication management, ordering, safe storage, prompting of medication to ensure compliance.
• Giving emotional support were required to allow service users to express their feelings, fears, and anxieties.
• Support by reassuring , monitoring, and assessing behaviors/mood change.
• Support with assessing and monitoring physical health needs.
• Engaging relevant professionals were necessary if any sign of deterioration is observed in any area.
• Support to maintain personal and environmental hygiene to continue living in a clean and safe environment.
• Support in maintaining a healthy lifestyle including meal planning, meal preparation, if need be, raise concerns with relevant professionals if need be.
• Help with booking , tracking, and reminding of appointments i.e., medical appointments or any other appointments to facilitate their move to permanent placements.
• Have a representative in any professional meeting regarding a step down service user if required to provide any information to help and facilitate smooth transition to their permanent placements.
