Service Overview - Homecare

Nurtrio provides flexible, person-centred home care services designed to support individuals to remain living independently within their own homes. The service is tailored to meet a wide range of needs and can be delivered on both a short-term and long-term basis. This includes support during periods of recovery, providing respite for informal carers, or meeting ongoing care requirements. Care is developed through personalised planning, ensuring that each individual’s preferences, routines, and desired outcomes are fully understood and respected.

The primary aim of the service is to promote independence, dignity, and choice while enabling individuals to live fulfilling and meaningful lives. Support is delivered in a way that aligns with each person’s daily routine and personal circumstances, ensuring a flexible and responsive approach. Nurtrio places strong emphasis on person-centred care, encouraging individuals to maintain control over their lives while receiving the support they require.

Care is delivered through flexible visit arrangements, ranging from short visits of approximately 30 minutes to longer periods of support, depending on assessed needs. Visits may occur once daily or multiple times per day, and care packages can be adjusted as needs change. An initial assessment is carried out to identify support requirements and to develop a bespoke care plan that reflects the individual’s goals and preferences.

Support provided may include assistance with personal care, such as help with getting up and going to bed, washing, dressing, and maintaining personal hygiene. Medication support is also available, including prompting or administration, as well as assistance with ordering and collecting prescriptions to ensure safe and effective medication management. Individuals may also receive support with nutrition and hydration, including meal preparation, assistance with eating and drinking, and access to services such as Meals on Wheels where appropriate.

In addition, the service includes support with maintaining the home environment through light domestic tasks such as cleaning and laundry, helping to ensure a safe and comfortable living space. Individuals are also supported to access their local community, including assistance with shopping, attending appointments, and participating in social or recreational activities. This promotes independence and helps maintain social connections and engagement with the wider community.

Emotional and social wellbeing is a key focus of the service. Support is provided to reduce isolation, encourage social inclusion, and assist individuals in accessing community groups and services. This may also include support for individuals experiencing mental health needs or living with long-term conditions, ensuring a holistic approach to care.

Nurtrio works collaboratively with individuals and, where appropriate, their families and other professionals to ensure that care remains relevant and responsive. Individuals are actively involved in the planning and review of their support, ensuring that care continues to reflect their wishes and lifestyle. The service supports a diverse range of individuals, including older adults, younger adults, people with physical disabilities, mental health needs, learning disabilities, and those with more complex care requirements.

Following an initial enquiry, a home visit assessment is conducted to establish needs and develop a personalised care plan. Care can typically be arranged within a short timeframe, allowing support to begin promptly. Overall, the service is designed to provide flexible, holistic support that enables individuals to maintain independence, dignity, and quality of life within their own homes.
