About Us
At Moks Care Ltd, we are committed to delivering services that are shaped by the people who use them. Our approach is based on co-production, person-centred care, and continuous engagement, ensuring individuals receiving support play an active role in designing, reviewing, and improving the service. We believe people using the service are experts in their own lives, and their voices are essential to delivering high-quality care.
Our management structure supports this approach:
Registered Manager:
Oversees person-centred policies, ensures staff training in co-production, and promotes a culture where service users’ voices shape service delivery.
Administrator:
Supports day-to-day service organisation and ensures service users are consulted when scheduling visits, activities, and care arrangements
Care Coordinator:
Ensures that digital systems are used to record service user preferences, track feedback, and maintain accurate care planning.
Field Supervisor and HR:
Ensuring only the best are employed and trained in order to maintain a high standard of care provision that is safe, responsive and effective. Maintains regular contact with service users during visits to ensure care is delivered in line with their wishes and identifies any changes required.
Together, the management team ensures that the views and preferences of people using the service are embedded in all aspects of service delivery.
Involving People in Choosing What Happens at the Service
Moks Care Ltd places the individual at the centre of care planning and decision-making. Every person who uses our service will be fully involved in the development of their personalised care and support plan. This process ensures that care reflects the individual’s personal goals, preferences, cultural values, and lifestyle.
Care planning meetings will involve the individual, family members or advocates (where appropriate), and relevant professionals. People using the service will be supported to express their views about:
How and when their care is delivered
Personal routines and preferences
Choice of activities and daily living support
Cultural, dietary, and religious requirements
