About Us
Transforming Support provides personalised supported living and community-based services for individuals with learning disabilities, autism, and other complex needs across the UK.
Supported Living is designed to enable individuals to maintain their independence while receiving tailored support within their own homes. Individuals hold their own tenancy agreements and are responsible for managing their bills and living expenses. Support may include assistance with setting up the home environment and addressing any maintenance or repair needs, depending on individual circumstances and regional arrangements. To support affordability, individuals may be eligible for a range of benefits and financial assistance. For those living alone, accommodation may also include a private entrance.
The provision of care and support is separate from housing arrangements. This means that any changes in the support provider do not impact the individual’s tenancy rights.
Individuals benefit from the security of a tenancy agreement, with eviction only occurring under specific circumstances and typically through a legal process. Our approach is centred on promoting independence, choice, and an improved quality of life through person-centred support.
We support individuals with learning disabilities, autism, and complex needs through a range of core services, including outreach support. Our model integrates Active Support and Positive Behaviour Support, alongside structured collaboration with local professionals and multidisciplinary teams.
Our highly trained staff work closely with each individual to regularly review outcomes and ensure that support remains aligned with their needs and aspirations. A strong understanding of what matters most to each person underpins our approach, enabling us to achieve meaningful and realistic outcomes in partnership with commissioners, external professionals, advocates, families, and wider support networks.
