Boughton Manor care home is registered with the Care Quality Commission as a ‘Care Home with Nursing’ for up to 27 Residents. We provide Long Term Care for people who are 18-64 years and 65 years and older with dementia and 65 years and over with Dementia, Mental Health conditions and long term physical conditions. Older adults with dementia and mental health conditions referred from Social Services and financed by the Local Authority and Health, and those who arrange care independently to fund themselves.

Our key emphasis is to ensure each person is identified as an individual who is unique to everyone else. People who use our services will receive care, treatment and support relevant to individual identified needs that are gathered during the initial pre-admission assessment. Through regular evaluation, reassessment and reviewing of individual person-centered care plans, enables changes to identified needs to be efficiently implemented to ensure that overall well-being is maintained. Further support and guidance from other professionals can therefore be sought if the need is identified. It is extremely important that the individual and their carer’s, family and friends participate in the review process to ensure that the individuals needs, choices, wishes and feelings are listened too, and appropriate action taken, of which we actively encourage and communicate at all times with individuals and their families.

Residents of Boughton Manor will benefit from a homely home which is ‘your home, our care’. The Residents will benefit from a highly trained skilled workforce, who are committed to providing high standards of care to meet individual choices, needs, requests and preferences.

A pre-admission assessment will be carried out to assess the suitability of the match between your needs and the Services and facilities of the Home. The suitability of your admission will be discussed with you and, if appropriate and with your permission, your representative(s).

Emergency admissions will be accepted only in exceptional circumstances, where the health or safety of the Service User is under threat, and normally only through a professional referrer such as Social Services. In this event, the suitability of the proposed Service User will be discussed with the professional referrer to ensure that the needs match the services offered, and the normal pre-admission assessment will be carried out within 48 hours of admission.
