Lifeline Support SERVICE
Day Centres, Home Visits, Community Outings, Care Home Visits
Lifeline offers a bespoke support service for all older adults in our community.
We also include a specialism in supporting people with dementia to promote social inclusion, activity and structure to the day.
Support is also provided for people with other long conditions and their carers.
Lifeline provides services in the North Nottinghamshire, Newark and Sherwood and Bassetlaw areas.
Areas covered include Retford, Worksop and surrounding areas.
Lifeline provides an in home and community support service led by a registered Community Support Nurse with extensive experience of working with older people, people with dementia and their families. This includes home visits, community support visits and day centre provision.
Tailored support is also available for adults living at home and wishing to access additional support.
A full assessment is completed for each family prior to introducing the support package.
Lifeline can prompt and support with tasks including personal care, continence support, medication administration and meal preparation.
Lifeline offers a 'Day Centre at Home' service to support people to participate in activity in the home such as life story and reminiscing, music, baking, gardening.
Lifeline also supports individuals to access their community with outings dependent on the individual's needs and interests. This can include accessing social groups, green spaces, cafe's.
