About Us

Community Care Team has been proudly delivering high-quality, person-centred care since 1992. We support individuals and families across Preston, Longridge, Chorley, South Ribble, and the surrounding communities, including Leyland, Penwortham, Walton-le-Dale, Lostock Hall, Bamber Bridge, Euxton, Whittle-le-Woods, Clayton-le-Woods, Astley Village and Buckshaw Village.

Our core mission is to enhance the quality of life for every Service User we support. We provide compassionate, reliable care for people living with illness, disability, or the challenges associated with later life, while also offering much-needed respite and reassurance to family carers. All staff receive comprehensive training, hold appropriate qualifications and are supported to deliver care of the highest standard whilst treating every individual with dignity, respect and kindness.

Our service is focused on helping people live safely, comfortably and confidently in their own homes. We provide support with:
* Personal care and dignity-focused assistance
* Incontinence care
* Meal preparation and nutritional support
* Medication assistance
* Companionship and emotional support
* Shopping
* Light household duties

We deliver our care by:
* Supporting vulnerable individuals while respecting their personal lifestyle and choices
* Working in close partnership with families, carers and healthcare professionals
* Operating to the highest standards, in line with all required policies, procedures and best-practice guidelines

About The Team
