Overview
Holt House is a home in the Milnrow area of Rochdale, which can accommodate just 3 residents.
We provide support for individuals aged 18 and over with Autism and/or Learning
Disabilities. Our service is tailored to meet the complex and varied needs of each person,
which may include:
Transitioning from Children's to Adult Services: Supporting individuals in developing
independent living ski ls and managing risk in preparation for long-term placements.
Behavioural Support: Assisting those who display behaviours that challenge, through
in-depth assessment and implementation of tailored strategies to promote positive
outcomes.
Dual Diagnosis: Supporting individuals with both Autism/Learning Disabilities and co
occurring mental health conditions, helping them to better understand and manage
their needs.
Post-Hospital or Interim Support: Providing structured, short-term placements for
individuals transitioning out of hospital settings while long-term placements are
being arranged, with a focus on preparing for successful reintegration into the
community.
Our model of care is based on delivering intensive, short-term assessment and
support, focusing on behavioural analysis, person-centred approaches, weekly
reviews, goal setting, and outcome achievement. We work colaboratively with
the multidisciplinary team to ensure a cohesive and holistic approach to care.
Our service provides a safe and structured environment that encourages
positive risk-taking, thorough assessment and review, effective risk
management, and the implementation of tailored strategies that meet each
individual's unique needs.

independence and improved well-being, and our aim is to support them in
reaching their fullest potential
