Diversity
We welcome the wide range in the community in our staff and clients and respect all culture, ethnic and religious views, and beliefs.
To ensure clients physical, spiritual, social, intellectual, emotional, and social needs are met to the best of our ability.
Family and friends are encouraged to be involved in the daily life and care plan of the client and are included in social and care needs.
Clients and staff are protected against abuse, intimidation, and discrimination and bullying by our zero-tolerance policy towards bullying and harassment.
Staff are supported by the management team to ensure we deliver this high standard of care, and that staff training needs are monitored, reviewed, and updated as needed or required.
Assessment process
New clients must be over 18 and fall into our categories of registration.
Prior to care starting a meeting will be arranged between the manager of care, the client and any family friends or advocate wishing to attend.
A Care coordinator will visit you and complete an assessment of your needs. They will carry out the assessment with you to identify what Support you may require, to meet your needs and to identify what you as a client need feel you. A care plan along with other forms will be left in your home for reference by your Care provider/s and yourself.
We will take a history of the client to include conditions which could affect care needs, recent falls, social and emotional needs, mobility, dietary and any other we or the client may find relevant to the client's care plan all these details will be completed on our assessment of need form.
A care plan will be written with you the client, risk assessment of the home and client and health and safety check list will be undertaken to protect the staff and client. The Care coordinator will carry out the basic health and safety Assessment to ensure that work can be undertaken safely in your home. Also, a Moving and Handling Risk Assessment will be completed to identify any equipment and/or handling techniques needed for care to continue to be provided. A copy of these assessments and a care plan will be left in your home for reference by your Support worker/s.
My carer’s clients report notes will be left with the care plan pack. This will be used by the Support worker to record information about their visit and a summary of tasks carried out and provides information for other Support workers, relatives, Doctors, and professionals.
Your Care Plan and Risk Assessment will be formally reviewed every six months or as requested by you, by the Manager to see if your needs have changed. However, Support workers will monitor your care plan daily and if at any time it is felt your needs have changed, he/she will let the Manager know as well and if you feel your needs change you need to inform the manager.
Care planning.
Care plans are tailored to suit the individual’s needs. Each client’s personal input into their own care plan is vital and the manager of care will discuss this. We believe in person centered care.
We will ensure our staff have the appropriate skills to provide this care, our staff work as a team to provide the best possible care, all staff are encouraged to take an active part in the care plan review process to ensure care plans are kept up to date.
Medication - our staff are only permitted to give medication from a Dosset box which has been filled by a professional. We would ask you not to embarrass the staff by asking them to give medication from boxes, bottles, or packets if there is not a MAR sheet in place, thank you. Staff may require extra training by the district nurse to apply certain creams, stockings and to assist with catheter and stoma bags.
