Details matter and our starting point is to fully appreciate each person as an individual with strengths, preferences and aspirations putting them at the centre of the process and enabling them to lead a full life.
This approach allows us to identify your needs and choices and how you want to be supported. We also ensure that we work with your family and carers as well as health and social care professionals to develop your support plan. Whether you just need 30 minutes per day or a live-in service, we are here to deliver the quality care you deserve.
Our aim is to give you more choice and control over how your care is delivered.
OUR STEPS TO QUALITY HOME CARE IN EALING ARE: -
1. INITIAL CONSULTATION
Please call or email us to arrange your initial chat. Our care manager will visit you for a free, no-obligation home consultation to discuss your needs and how we would be able to help.
2. YOUR TAILORED PLAN
Following the initial meeting, our care manager will document your needs and devise a support plan that best suits you and will discuss any changes you would like to make.
3. MEET YOUR CARE TEAM
We will then introduce the members of your Care Team to ensure that you are happy with the people who will deliver the service, before they start working for you.
4. ON-GOING SUPPORT
Throughout your time with us, we believe that continual monitoring of what we do and, more importantly, communication with you, is vital to ensure that you get the service you require.
